Application and Consent FormsPlease print these documents in order to complete them. Once completed please send ALL forms, along with a copy of your CDL, your Long Form DOT Physical and your Social Security Card to us by fax at (612) 436-0265 or you can scan your documents and send them to us for processing via E-mail to Safety@tcompanies.com.

- Your Application WILL NOT BE PROCESSED unless ALL QUESTIONS are answered.
- We need 10 years of Employment History - NO Gaps!
- PLEASE, double check your phone numbers and addresses for accuracy - It will avoid delays in processing.
- Please Use "N/A" if a question does not apply to you.
- Enter dates as follows: mm/dd/yyyy
- Enter Social Security Number as follows: 123-45-6789
- Enter phone numbers as follows: 123-456-7890

Once Completed please fax all of the requried documents (Application, Consent Forms, CDL, DOT Physical, and Social Security Card) to (612) 436-0265 or you can scan them and Email them to: Safety@tcompanies.com.
